I understand and agree to the minimum required options listed below of Children Habilitation Center's Registry as follows: Position Requirements I agree that I will comply with CHC RT's job description faithfully and carry out the duties and responsibilities communicated to me, CHC managers/director. I agree to always comply with all company policies, rules and procedures. I understand CHC may also add other duties within a reasonable scope to my role. I agree to attend and participate in all related department and/or position specific orientation(s), required meetings, pre-test and procedures as directed by the RT Director, Administrator, and DON. I agree that CHC has the right to supervise my work and take disciplinary action if I violate any policies. I agree to comply with Company's HIPAA policy and non-compete agreement stated in handbook. Schedule & Attendance I agree to be available to work the following: A minimum of three (3) shifts within a one-week posted schedule. I agree to work a weekend rotation that will be included as (2) days towards the (3) shift requirement. I understand that if I do not work; A minimum of three (3) shifts within a one-week posted schedule this may be considered a voluntary resignation. Thanksgiving Day Memorial Day Christmas Day Independence Day Juneteenth Good Friday MLK Birthday New Years Day Labor Day I agree to work holiday shift based on operational needs. All holiday trades should be communicated and approved by the RT Director or his/her designee. If an employee-partner calls off on a holiday, the employee-partner will be required to work another holiday to be determined by the RT Director. JOB SUMMARY Respiratory Therapist provides treatment and diagnostic respiratory services . This position requires clinical assessment skills and the ability to make appropriate clinical decisions according to the Children's Habilitation Center established guidelines and state regulations. RESPONSIBILITIES Complete comprehensive resid ent assessment s and review medical record s to recommend any clinical changes in clinically indicated therapy ; relays any identified issues to the Charge Nurse Participates in physician and nursing Rounds Perform and/or teach airway clearance and volume expansion modalities for the purpose of assisting resid ent s in the mobilization of secretions or increasing lung volume Administers aerosol therapy , oxygen thera py, and humidification per physician order Perform procedures related to the care and maintenance of an airway M aintains and weans mechanical venti latory support of resid ents per physician chest orders Applies, assists, and monitors resid ents using Passy Muir speaking valves Performs / monitors standing orders for oximetry and capnometry as indicated Administers respiratory medications as prescribed Accompanies resid ents on emergency transport via ambulance Responds to Code Blue situations and is a participant in the Code Blue team Performs complete and accurate documentation within the electronic medical record Keep s up to date with departmental information disseminated through various media Verifies all equipment is in safe operating condition and reports any problems noted to the charged nurse. Orient s new staff to the working of the department and services provided Actively participate s in evaluation of products and therapeutic modalities Performs other duties as assigned. Demonstrates appropriate interpersonal skills to work productively with resid ents, families and co-workers. Compliances with all CHC policies and IDPH regulations Transport Ventilator dependent residents QUALIFICATIONS Current Respiratory Licensure issued by the State of Illinois (1) One - year respiratory therapy experience; preferably in a pediatric setting Current BLS certification ( American Heart Association ) WORKING CONDITIONS Usually works in a well-lighted, well-ventilated area May be exposed to potentially infectious diseases I understand that the schedule provided to me must be adhered to. Failure to follow schedule will be considered a voluntary resignation. Any schedule change request must be submitted no later than 2 week prior to the following day of request. It is my responsibility to submit any request via e-mail to the scheduler and to keep a copy of my schedule for my records. Children Habilitation Center reserves the right to cancel Registry personnel up to two (2) hours before the start of a scheduled shift. I agree to notify the appropriate personnel at Children Habilitation Center if I am unable to work a scheduled shift by calling in at least (3) hours prior to the start of any shift. I agree to submit a minimum two-week notice prior to a voluntary resignation, and to work all shifts as scheduled during the notice period. I understand that if a regular position becomes available that I must submit my request to be considered for that position in writing to the Human Resources department. I understand this request does not guarantee that I will be placed in that position. 2b. Attendance Policy Providing adequate levels of competent staff is essential to meet the stated mission of Children Habilitation Center and excellence in patient care. Any requests off/ call offs for any scheduled shifts including weekend rotation, he/she may be required to work another weekday shift/ weekend shift, based upon the needs of the department and at the discretion of the Respiratory Director. If you fail to work the make-up shift, you will be subject to disciplinary action, up to and including termination. If an employee-partner has an excessive amount of absences, tardy, and/or call-offs, they will be subject to disciplinary action, up to, and including Compensation As compensation for the services provided, the Registry Employee shall be paid a wage of $___45.00_______ [per hour] and will be subject to performance reviews as needed. All payments shall be subject to mandatory employment deductions (State & Federal Taxes, Social Security, Medicare) I agree that I cannot participate in any of the Children Habilitation Center's benefits under this registry agreement. I understand that overtime will be paid only after 40 hours have been worked in one scheduled work week. I understand that upon acceptance/converting into a regular full-time or part-time position, pay may be adjusted to accommodate the company's cost to offer benefits for the employee-partner.
