Job Title: RN Care Coordinator/Triage Description: FT 40 Hours , Home-based Day Hours Compensation: Excellent Pay, based on experience RN -- Care Coordinator/Triage -- 40 hrs./week Locations: Stillwater/Remote Join Midwest Spine & Brain Institute -- a growing and innovative spine and brain surgical practice specializing in the treatment of spinal injuries, disorders, and research. We are currently seeking a Care Team Triage RN at our Stillwater Central Business Office location/remote. Please note that this will be an in office position until fully trained and then will transition into business office/home office at that time. Responsibilities include: GENERAL SUMMARY OF DUTIES: Provide prompt, courteous telephonic and patient portal triage and care management for Care Team patients. Build and foster positive relationships with Care Team providers and patients. Assist patients in understanding plans and navigating their care. PRINCIPAL DUTIES AND RESPONSIBILITIES: Communicate with patients via phone and patient portal. Provide outstanding customer service by responding to patient calls and concerns same day. Provide peri-operative education, assessment and counseling. Implement orders based on condition specific protocols. Monitor compliance with therapeutic plans. Collaborate with team members, department team leads, supervisors, management and providers to develop department goals that are in alignment with company goals. Utilize the nursing process to ensure quality patient care. Understand and evaluate Care Team and interdepartmental work flow and processes and identify opportunities for improvement. Participate in quality improvement activities. Measure effectiveness of quality improvement activities. Participate in development of protocols, policies and procedures for Care Team Compile department statistics. Assist in compliance with regulatory agencies (OSHA, Department of Health, DEA, etc.) and appropriate documentation and record keeping as it applies to the Clinical Care Team Establishes and maintains partnerships with external customers and vendors. The ideal candidate will have excellent attention to detail, effective decision making and problem solving skills, and have the ability to work well as part of a team. This position works collaboratively with surgeons, mid-level providers, and multiple departments. Experience with NextGen EHR/EPM and EPIC is desirable. Excellent Pay and benefits package.
