Korean Women's Association (KWA) is seeking Community Health Care Coordinators in Lynnwood (Snohomish County). Full time position, rate of pay $19.00 - $23.00 (DOE) per hour plus benefits. Purpose of Position Provides comprehensive navigation and support services to clients related to Pathways care coordination, Health Homes care coordination, and/or Cancer Screening Navigation. Collaborates with licensed providers, health insurance companies, and administrative staff in multiple organizations to help clients access person-centered and culturally responsive services throughout the health and behavioral health care system. Essential Duties and Responsibilities Develop partnerships with culturally diverse organizations, and conduct targeted outreach to those Medicaid/ Apple Health recipients who are either high risk, high utilizers of health system, or have chronic health conditions. Provide one-to-one prevention education and information about the care coordination program, tailored for different levels of health literacy. Conduct comprehensive needs and risk assessment. Identify priority areas of assistance in collaboration with the client and share the information with health care providers (in HIPAA compliant way, with client's consent). Help clients develop Healthy Action Plan (HAP) and increase Patient Activation Measure (PAM), using Motivational Interviewing and client engagement skills. Provide warm referrals and direct access to translators/ interpreters, health and food benefits, transportation services, affordable housing shelter, domestic violence shelter, immigration & naturalization services, employment training & services, mental health and chemical dependency treatment, and other services. Introduce pregnant clients to healthy nutrition, healthy activity, emotional support, parenting education, and other programs that help the client deliver a healthy baby. Communicate information to clients in a culturally responsive way that is easily understandable and emotionally supportive – via in-person, email, phone, and text. Capture accurate and timely information about the clients' clinical and healthy activities on an ongoing basis, in a secure system. Attend training as required. Drive clients safely to medical clinics, hospitals, behavioral health providers, and social service agencies as necessary. Performs other duties as assigned. Position Requirements, Competencies and Qualifications Satisfactorily pass criminal history and reference checks. Current driver's license and auto insurance. Proficient in MS Office Suite, e.g. Excel, Word, Outlook, etc. 3.1 Education Associate's degree in social services, health, behavioral health or related field or combination of education and experience. 3.2 Experience 1 plus years as care coordinator, community health worker, navigator, counselor, or related role, preferred. Experience in gathering, tracking, reporting and entering data for various types of reports via computer systems, verbally and/or presentations. Experience in handling information in a HIPAA-compliant manner 3.3 Unique Expertise/Certification/Registrations One of the following is required: Completion of WA. Dept. of Health 8-week Community Health Worker training; OR Completion of Pathways certification training.
