Our Company is looking for a Registered Nurse ( Home Health) to join our team. T his role will be responsible for providing Home Health care to patients in the local area in the comfort of their own home, He/she will administer medication, monitor and record patient progress, and educate families and the patient as needed. The ideal candidate will display compassion, education, proficiency and experience while assisting the patient. This critical role assists in creating and managing nursing care plans and will participate in the evaluation of current and future medical needs of the patient. Responsibilities: Provide exceptional patient care – Maintain nursing standards while supporting patients with all applicable medical needs. Create and provide patient care plans as needed or requested. Communicate with the patient, their family and members of the medical team to achieve the desired goals of the patient-care plan. Conduct administrative duties – Attend medical conferences and courses for ongoing care and education. Assess patient care plans and identify areas of improvement to ensure quality of care. Top Skills Required:• 1+ year of professional experience as an RN (Home Health experience preferred)• Active RN License in the State of IL• Associates Degree (or higher) in Nursing• BLS and CPR Certification (if you don't have this - we will help you get it!)• Valid Driver's License, Reliable Mode of Transportation, and Auto InsuranceWhat you need to know:• Fulltime | Monday - Friday• Flexible schedules, no long shifts, and minimal weekend shifts a year• A comprehensive 6-week RN onboarding program•Travel Position (Travel Reimbursement provided)• Full Benefits (Health, Dental, and Vision)• $15K Bonus (Paid Out Quarterly) **This is a Pay Per Visit position About Company: Residential Home Health and Hospice is a nationally recognized leader in home health and hospice for over 20 years and has been named a Top Workplace 12 years in a row.
