HOURS - Full time Weekdays Who We Are The work of Good Shepherd is based on a fundamental belief in providing hope and restoring dignity to the most vulnerable members of our community. Our services include emergency food transitional housing and education for homeless and street-involved youth; hospice palliative care; community mental health programs; and supportive housing programs. We are committed to integrating Harm Reduction practices into our programs and to prioritizing the rights, dignity, and choices of those who use substances. By integrating a Harm Reduction framework at Good Shepherd, we can decrease barriers to service and better support some of our community's most marginalized members. This opportunity is with our Good Shepherd - Barrett Centre How You Will Make an Impact The successful candidate will extend our mission to patients of Shelter Health Network and HAMSMa RT through the provision of respectful and appropriate system navigation and advocacy services. This may include developing skills, education, outreach, and case conferences. The successful incumbent must be able to work independently and cooperatively in a challenging and rewarding values-based environment. Partner with clients HAMSMa RT and Shelter Health Network in developing and executing a plan of care that reflects client priorities and needs Establish positive rapport with clients and provide individualized home/community-based supports geared to building optimal health, wellness, safety and inclusion for clients Facilitate timely client access to community services and resources including those related to health and wellness, appropriate/affordable housing, income support, food security, therapeutic recreation, faith or cultural activities etc. Provide referrals, advocacy and system navigation services that address client needs and goals Responsible for participating in Good Shepherd's Health and Safety programs, procedures, procedures and work in compliance.
