Complete application CV Up-to-date transcript Job Purpose:The PGY1 Community-Based Pharmacy Residency, is an intensive one-year program designed to cultivate pharmacists who will lead the transformation of community pharmacy practice. This residency provides a comprehensive learning experience that prepares residents to deliver exceptional patient care, assume leadership roles, and refine and expand their clinical, leadership, and educational proficiencies. Focused on health-system specialty pharmacy practice, the residency program aims to develop proficient pharmacists skilled in managing complex medication regimens and providing comprehensive medication therapy management. The curriculum offers hands-on experience in clinical pharmacy, pharmacy operations, leadership, and teaching, equipping residents to significantly contribute to community health and innovate in pharmacy practice. Emphasizing continual improvement, lifelong learning, and professional responsibility, the program prepares residents for diverse roles in patient care, healthcare collaboration, and educational initiatives, fostering a commitment to ethical practice and healthcare advancement. Through these comprehensive experiences, residents develop into confident, autonomous pharmacists who adeptly apply their knowledge, effectively utilize resources, and confidently leverage technology to improve the quality of patient care. Residents will learn to forge connections with patients and prescribers, enhance medication adherence, and promote evidence-based care in a community pharmacy setting with a specialized focus on health-system specialty pharmacy practices. The PGY1 Community-Based Pharmacy Resident will report to the PGY1 Residency Program Director. Research project : Residents engage in a comprehensive year-long research project, mentored by experienced preceptors, to deepen their knowledge and skills. This initiative offers the chance to showcase their findings and innovative solutions at various organizational, local, state, or national conferences, providing a platform for professional development and contribution to the broader field. On-Call : Residents will participate in an on-call rotation, providing critical support and decision-making assistance during non-standard hours. This role is essential for maintaining continuous, high-quality patient care and responding to urgent healthcare needs. Staffing : In addition to regular responsibilities, residents may be required to perform staffing duties. This may include covering shifts in various pharmacy settings or assisting with unexpected workload increases, ensuring seamless pharmacy operations and optimal patient care. Responsibilities: Deliver exceptional patient care based on evidence-based guidelines while innovating services to enhance community pharmacy practice. Build collaborative relationships with healthcare professionals to improve patient care and healthcare delivery. Engage in research and scholarly activities, including evidence dissemination, to contribute to the advancement of the pharmacy field. Promote health literacy and maintain involvement in quality and safety improvement projects within the pharmacy. As applicable, mentor and instruct pharmacy students and technicians, fostering their development through educational and practical experiences. Job Duties: Reviewing and interpreting clinician's patient care note/assessments for supportive clinical data needed for prior authorization approvals and development of care plan recommendations. Develop and refine disease and drug specific clinical protocol to maximize patient adherence and promote evidence-based cost-effective care. Monitor on-going patient adherence (PDC reports, patient assessment data, and mobile platform data) across assigned site(s) and disease categories with focus on oncology and other specialty specific disease states. Evaluation of EMR to review: drug and disease interaction, medication selection, labs/vitals, allergies, RX orders/renewals and direct MD communication, and other relevant information important to maximize medication adherence and optimize patient outcomes. Recommending alternative therapeutic drug regimens and disease specific treatments according to guidelines as appropriate. Monitor and evaluate therapies for side effects or other medication related issues, recommend drug therapy changes to prescribers as appropriate. Medication therapy management (MTM): including reviewing and monitoring of medication regimens, advising clinicians on evidence-based therapy as indicated, documenting interventions; assessing patient adherence rates. Clinical interventions including but are not limited to comprehensive pharmacist consultation, motivational interviewing/coaching, drug regimen optimization, medication reconciliation, medication lists or reminders, medication synchronization, coordinate home nurse visits, transitions between liaison/PSC, use of mobile app or other technologies. Productivity monitoring and reporting weekly stats for growth development. Identifying and resolving insurance related issues, recognizing situations where financial assistance can be applied. Supervising communication of support personnel compliant with HIPAA rules and regulations. Maintaining registration (when applicable) and all remote pharmacist activity to comply with local, state and federal drug laws as regulated by each state board of pharmacy and the DEA. Direct communication with other health professionals as necessary regarding patient care. Develops own knowledge by participating in education programs; training pharmacy staff, students, interns, externs, and health care professionals. Manage, organize, and update relevant data using database applications. Apply observations and recommendations to operational issues, increased productivity, quality, and customer-service standards. Assist with research projects and peer reviewing articles as needed. Identify trends, resolves problems; recommend improvements; implements change. Actively participates in process improvement initiatives. Protects organization's value by keeping information confidential. Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations. Willing to do whatever is necessary for the betterment of the Company. Other duties as assigned. Skills : Analyze information, exercise good judgment, strong verbal and written communication skills, general knowledge of specialty medication pharmacology, local/state/federal pharmacy regulations, able to manage processes, highly productive, quality focused and attention to detail. Ability to critically evaluate clinical data and literature to improve patient care and service delivery. Proficient with standard software used in the healthcare office setting (Microsoft Word, Excel, and PowerPoint). Ability to identify barriers to medication adherence and implement and monitor care plans to improve medication adherence. The ability to motivate, inspire and empower patients to achieve better outcomes and identify disease-specific goals of therapy. Location: Hybrid - onsite (Worcester, MA) and remote rotations Experience/Education: Required Licensure/Certification : Residents are licensed pharmacists in the state(s) of Massachusetts within 120 days after the program start date. Any time lost after the 120 days have past, must be compensated for by extending the residency beyond the originally planned end date, as decided by the Residency Steering Committee, their employment may face termination. However, circumstances outside the resident's control that prevent licensing will be considered individually for termination decisions. Experience : Graduate of an Accreditation Council for Pharmacy Education (ACPE)-accredited (or ACPE candidate status) school of pharmacy. Applicant must hold a Doctor of Pharmacy (PharmD) or equivalent advanced degree and be eligible for a pharmacist license.
