Are you a Pharmacist, looking for a new challenge Service Care Solutions are recruiting for remote Pharmacist with experience in working in Primary Care. This role is for 37.5 hours per week available, Monday to Friday from 9am-5pm Job Purpose: Pharmacists Remote Pay Rate: Up to £25 per hour Location: Remote Working Hours: Monday to Friday, 09:00-17:00 Key Responsibilities: Working alongside a multidisciplinary team to help deliver outstanding patient care Reviewing patient’s acute medicines requests Liaise with clinicians and administrative staff to resolve medicine related queries Liaising with local chemists to solve medicinal problems, shortages, and other issues Managing long term conditions such as asthma and COPDCarrying out medication reviews including scrutinizing polyshop patients Perform medicines improvement work to maximise safe, cost effective best practice in prescribing under the supervision of the board or Clinical Pharmacist/s Assist with medication safety monitoring systems e. g. high risk drugs Monitor practice prescribing and adherence to their employer and the PCN’s policies, included but not limited to any other local and national prescribing policies and guidelines Undertake a wide variety of audit work to analyse, improve and rectify prescribing behaviours The successful candidate will hold a suitable Pharmacist qualification and GPh C registration and will have experience working within Primary Care.
