The Role: We offer a wide range of services to support our residents with mental health problems, and in this important role you'll join the Health and Mental Health Adult Social Care team that's responsible for delivering exceptional care and assistance to our service users and their carers. You will have excellent knowledge of the Care Act 2014 and will use this statutory framework to coordinate care and support that is designed to meet and improve the individual requirements of everyone who depends on us, and carry out strength: based backssments that help people choose personal, need: specific outcomes. But your work won't stop here : you'll also deliver integrated health and social care by supporting adults with serious mental health issues as they focus on their recovery, and help them to identify risks while ensuring that there is an appropriate protection plan in place. We'll also expect you to coordinate the establishment of personalised safeguarding, and work towards agreed health and social care performance indicators and excellent standards of practice.
