Summary Community Based Day Services Professional is responsible for creating and implementing meaningful learning options and community experiences for Participants. The position is also accountable for documenting participant progress, concerns and/or other participant related data, according to agency policy and procedures and for making sure that participants gain greater self-satisfaction and/or independence as a result of his/her participation. Duties Assist participants in achieving agreed-upon individual program goals that move him/her to the next level of independence. Document (according to details associated with the goal), on a timely basis, assigned participant progress toward goal attainment. Additionally, the position is responsible for completing other participant related data according to agency policy. Position is responsible for initiating, designing and implementing a variety of community opportunities and creative class options for assigned participants. Help participants identify things that they want to try, experience, work on or achieve. Encourage and assist participants with experiencing new things so they have a broader array of experiences from which to make their choices. Serve as a role model for all participants by interacting with all people in a calm, fair and respectful manner. Communicate with the program Manager regarding participant goals, activities, and concerns. Consistently work as a cooperative team member with all participants, peers, and supervisors to ensure goal implementation. Complete all employment-related reports/forms according to agency policy (i.e. In-Service Training forms, Accident Reports etc.). When driving RCOC vehicles and/or transporting participants, follow safe and lawful driving standards. Report vehicle related data according to program Manager. Be active in keeping calendars, goals, and supply lists updated; checking and filling supplies; cleaning and organizing items and spaces used by the department. Create and provide sensitive, respectful, meaningful and imaginative learning possibilities and community experiences for program participants that include opportunities for participant personal growth and self-determination. Complete assigned responsibilities, reports, employment-related report, vehicle logs and all participant related documentation in a timely manner, according to agency policy and procedure. Work cooperatively with all participants, peers and supervisors in order to ensure program integrity and cohesive service delivery. Continuously follow agency policy, procedure and mission. Requirements Ability and willingness to work outside in varying weather conditions (wetness, snow, dust, noise, hot/cold etc.) Willingness to access the community throughout the year, with participants in a variety of ways including the city bus, walking to site, etc. Willingness to work with people with an array of disabilities including physical limitations. High School diploma of equivalent required. Two years? experience working with adults who are developmentally disabled, or in a related field, or two years of related education, preferred. Ability to independently and spontaneously travel to various locations. Have a valid Wisconsin Driver?s License and good driving record. Drive agency vehicles as required. Pass a Caregiver Background Check Pre employment Drug Screen
