Your Specific Responsibilities: The incumbent is part of a global public health organization and is expected to support the Florida Department of Health in Manatee County in its mission to protect, promote and improve the health of all people in Florida through integrated state, county and community efforts and its vision to be the Healthiest State in the Nation. Furthermore, the incumbent is expected to incorporate the agency values of Innovation, Collaboration, Accountability, Responsiveness and Excellence in all aspects of their work. The incumbent works within the community to expand the education and testing program to target populations for HIV/AIDS. He/she works closely with various community organizations/schools to help increase HIV awareness thru presentations, health fairs, or other events per supervisor approval. The incumbent will work closely with the Operations Services Manager to staff direct outreach activities throughout the community. This may include, but is not limited to, distributing condoms, and visiting local businesses for the purpose of making literature and condoms available to the local community. He/she will assist with clarification, resources development, and advertising of special events which will include STD Prevention Month, National Testing Day, National Latino Awareness Day, World AIDS Day, Black Awareness Day, and National Girl and Women Awareness day. The incumbent networks with existing indigenous organizations to provide a comprehensive and culturally sensitive outreach programs within specific cultural/ethnic minority populations where HIV has taken a disproportionate toll on the community. Attends community meetings addressing HIV/AIDS issues as assigned. Will act as Team Lead to the HIV/STD Outreach Team. Duties will include: maintaining daily activities in the department and ensuring staff coverage at mobile outreach events and the jail, conducting daily huddles to inform staff of daily assignments and share any DOH updates with the team, documenting total number of tests conducted at each outreach event and reporting totals to the division director daily, reviewing the annual workplan and ensuring activities are being met, ensuring the mobile unit is in good working condition, attending community meetings, and keeping the division director apprised of any issues or concerns that inhibit the success of the HIV and STD Outreach Team. The incumbent will ensure that the patients are knowledgeable about such programs as Housing Opportunities for People with AIDS (HOPWA), the AIDS Insurance Demonstration Project (AICP), the AIDS Drug Assistance Program (ADAP), Ryan White Title II, PAC Waiver and HIV/AIDS Case Management. In addition, the incumbent assists patients on a limited basis to access social programs such as disability, food stamps, emergency food baskets, clothes, locating affordable housing and any other needs related to the patients holistic care. If the clients have a HIV case manager these services should be coordinated with their manager. The incumbent works with patients to develop individualized plans of care to encourage compliance with their treatment/medication regimen and laboratory test.
