Acenda's expert team helps each individual, family and community move forward in their journey towards health and happiness.Key Responsibilities: * Travel to clients' homes and provide home visits to first-time mothers providing education and support through pregnancy and up until the child's 2nd birthday. * Work to improve pregnancy outcomes, child health and development outcomes, and the economic self-sufficiency of families. * Build long-term therapeutic relationships with clients and their families. * Provide community resources as needed per nursing assessment. * Adheres to the nursing process and the Nurse-Family Partnership model of home visitation. * Attends and participates in case conferences and team meetings. * Collaborates with client and family to develop short- and long-term goals. * Formulates nursing diagnosis based on nursing assessment, client goals and Nurse-Family Partnership outcomes.Requirements: * Bachelors degree in Nursing (BSN) required * Active NJ Registered Nurse license (in good standing) * Must have and maintain a valid driver's license, use of an insured vehicle and an acceptable driving record * Professionalism and a commitment to excellence in careWe provide: * $3000 Sign-on Bonus * All expenses paid week-long trip to Denver, Colorado * Flexibility with your work setting and hours * Competitive salary and 401k with a match * Year-end performance bonuses * Health, vision and dental coverage * Generous number of vacation days each year * CEU's and free training programsAcenda's Equal Employment Opportunity CommitmentAcenda Integrated Health is an equal opportunity employer, and all qualified applicants will receive consideration for employment without regard to race, color, religion, age, sex, national origin, disability status, genetics, protected veteran status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state or local laws.
