Incumbent also notifies pharmacist of any discrepancies between patient problem list, medications from home and medication list in the EMR. Responsibilities: Successfully performs medication reconciliations. Identifies patient safety issues within the medication reconciliation process Maintains ED floor stock medications/supplies in ECC Pharmacy. Communicates with team members and other care providers regarding assigned duties. Acquires age-specific knowledge necessary to provide appropriate service and communications. Maintains a good and current working knowledge of the clinical computer system. Participates in maintaining designated equipment and work areas. Takes an active role in self-development. Performs other duties as needed.
